As patients see more doctors with various specilities, it is critical that all members of the healthcare team understand what medications the patient is taking. Admission medication reconciliations help the healthcare team understand what medications the patient should be taking as well as how the patient is actually taking their prescriptions. Proper understanding of a patient's prescribed medications, OTC products, and herbal supplements help create a continuation of care and ensure proper monitoring of drug/drug interactions and compliance.
